canning stew

i cooked up a batch of stew a few days ago.in which it didnt last..one reason for it,is i didnt preasure cook it long enough.but yet i been told 2 diffrent reasons by 2 diff ppl to why else it went bad on me.one said its because of the stew meat.and the other said carrots..what other reasons is there?
